• Evaluates the need for medical social casework services to patients in their place of residence.
• Communicates patient needs to the physician and home health care team.
• Plans quality medical social work services.
• Implements social work services to patient in their place of residence.
• Evaluates medical social work services plan on an on-going basis.
• Maintains assessment and progress documentation accurately.
• Participates in the quality improvement program.

• Master Degree in Social Work from a school accredited by the Council on Social Work Education.
• Minimum one year social work experience in a health care setting.
• Previous experience in a licensed home health agency preferred.

• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
